The second Nancy is another I-jin created from Mata Hari and physically identical to the Nancy who works beside Yomiko. She remains with Ikkyu while the other Nancy infiltrates the British Library, with handedness providing the tiny physical difference that eventually makes the final deception possible.
During the attack on the I-jin fortress, the two Nancys fight and the surviving woman aboard the rocket is initially assumed to be Ikkyu's partner. She is actually Yomiko's Nancy, who has switched her weapon to the other hand to impersonate her counterpart.
The Nancy left behind survives the fortress battle but suffers severe memory loss after oxygen deprivation. At the end of the OVA she is being cared for in a secure facility and knows the dead Nancy only through the story Yomiko tells her about an "older sister."
She is not a replacement created after the climax and she is not a child for Yomiko to raise. She is the surviving half of the OVA's two-Nancy reveal, left alive without the memories that made the distinction between them matter.

Two Mata Hari I-jin · One Small Difference
Nancy Makuhari is an I-jin derived from Mata Hari, but she is not the only one. Ikkyu has two physically identical Nancys in play, which is why the apparent betrayal and the fortress confrontation are so difficult for Yomiko, Ikkyu and the viewer to untangle.
The small physical tell is handedness. When the two Nancys finally fight, the survivor is able to exploit that difference by deliberately using the opposite hand and allowing Ikkyu to believe the wrong woman won.

Why it matters
The clone reveal is not there just to produce a surprise second Nancy. It lets the OVA turn identity itself into the final piece of espionage. Ikkyu created an almost perfect duplicate and then dies because he believes he can tell which one belongs to him.

Yomiko's Nancy returns after Ikkyu seemingly killed her and confronts the other Mata Hari clone. Both women have the same phasing ability, making ordinary attacks difficult to land and forcing the fight into a contest between two people built from the same template.
